On December 11, ACL posted a Notice of Funding for chronic disease self-management education (CDSME) programs for older adults and individuals with disabilities. The purpose of this funding opportunity is for applicants to develop a statewide initiative to address the capacity for, and delivery of, evidence-based CDSME and support programs. Additionally, this funding is for the development and disseminate of 508-compliant resources specific to the grant learnings to enhance knowledge in serving the target population and aid in the sustainability of programs.
