Request for Proposals - Ohio Access Success Project

The Ohio Department of Job and Family Services (ODJFS) released this Request for Proposals (RFP) for the purpose of identifying a vendor to develop and implement the Ohio Access Success Project. This is
a nursing facility transition pilot project in which targeted individuals who are aged or cognitively, sensory or physically disabled and who have been residing in a nursing facility have an opportunity to choose to transition from that setting to a community-based one.
