The development of this Best Practices Manual is a pilot project to demonstrate a step-by-step process of re-integrating long term nursing home residents from residential living back into the community. This innovative Project is the result of an initiative made possible under the Passages Program sponsored by the Arkansas Division of Aging and Adult Services, in 2000-2003.
